Form MI-4642 print requirements

To print form MI-4642, Michigan Voluntary Contributions and Organ Donor Registry Schedule:
  1. Make sure there is a total contribution amount populated on the form:
    1. Go to
      Tax Forms
      States
      Michigan
      . Select
      Resident
       or
      Part-year Resident/Nonresident
      .
    2. Select
      MI-4642 - Voluntary Contributions
      .
    3. Make sure there is an amount greater than 0 on
      Line 6
      .
  2. Make sure the donor is selected:
    1. Go to
      Organizer
      States
      Michigan
      Contributions
      .
    2. Go to the
      Donor
       section.
    3. Make a selection besides
      Not applicable
      .
  3. Do a full recompute, then print.
If the form doesn’t print, make sure the correct extension option is selected:
  1. Go to
    Organizer
    Payments and Extensions
    Extensions
    .
  2. Go to the
    State Options and Payments
     tab.
  3. Make sure
    option 1
     or
    option 2
     isn't selected for
    Michigan
    .
  4. Do a full recompute, then print.
