To generate and print Ohio Form IT-Wavier, Ohio Income Tax and School District Income Tax Penalty Waiver Request:

Go to Organizer
, select States
, and then Add States/Cities
.

For state Ohio
, select the residency option.

Go to Organizer
, select Payments and Extensions
.

Select Extensions
, and then the State Options and Payments
tab.

For state Ohio
select options 3
, 4
, or 5
.

Go to Organizer
, select States
, and then select Ohio
.

Select E-File
, in Additional information
and go to E-file Opt Out
section.

Select or unselect print opt out form
.

Do a full recompute, then print.
