Go to the row for Kentucky.
In
Column I - Date qualified in State
, enter the date in YYYY-MM-DD format (for example, 2021-02-03).

Don't use MM/DD/YYYY or YYYY/MM/DD format.
If the entity isn't a foreign corporation, remove the date from
Column I - Date qualified in State
for Kentucky.
Do a full recompute, then recreate the e-file.
