Screen 9465 - Installment Agreement Request (1040)

Overview

Use this screen to enter the information required to request a monthly installment agreement from the IRS to pay a tax balance due. The application uses this information to complete Form 9465, Installment Agreement Request.
You can choose a custom paragraph to include Form 9465 signature and filing instructions with the filing instructions for the return. Different paragraphs are available, depending on whether the return is an original or amended one. To choose a custom paragraph on a client basis, access the Letter screen. From a

for an original return or

for an amended return. To print a custom paragraph for all clients, choose Setup and then 1040 Individual, select the Letters & Emails button in the Federal tab, select the Custom Paragraphs tab, and mark the
Form 9465 (FI)
and/or the
Form 9465 Amended (FI)
box.

Form 9465 - General Information

When this field is marked, the application prints Form 9465 (2159 or Notice) and includes the amount from the
Total amount owed shown on notice
field, regardless of whether the 1040 return has a balance due. Form 9465 is treated as filed separately from the return, prints separate filing instructions, and does not include Form 9465 in any electronic files.
If Form 9465 is being filed in response to a notice from the IRS and applies to a different tax form than the one filed in the current year, enter the name of the form. Enter the name exactly as it should appear, including using the word "Form" to print at the top of Form 9465.
If Form 9465 is being filed in response to a notice from the IRS and applies to a different tax year, enter the year here. This information appears at the top of Form 9465.
Form 9465 instructions include two different sets of mailing addresses for Form 9465 when it is being filed separately from the tax return. One set of addresses is for taxpayers who filed Form 1040 with Schedule(s) C, E, or F for the tax year for which the installment agreement is being requested. The other set of addresses is for all other taxpayers. Mark this box if the taxpayer filed Schedule(s) C, E, or F with Form 1040 for the tax year of the installment agreement request to print the correct mailing address on the Form 9465 Filing Instructions and 9465 Slipsheet.
If Form 9465 is being filed in response to a notice from the IRS, enter the amount owed that is shown on the notice.
If you are electing to allow the IRS to make an electronic funds withdrawal for a portion of the tax due on a return by entering the appropriate information in the ELF and Bank screens, the application defaults to treating that payment as the amount paid with the tax return. Otherwise, enter the amount paid (if any) with the Form 9465 submitted with a tax return or IRS notice. To divide the payment between a direct debit amount and a check amount, enter the amount for direct debit in the
Amount of tax payment (Force)
field in the ELF screen, and enter the total amount paid with the return in the
Amount paid with the tax return or notice
field on the 9465 screen. When printing filing instructions as if the installment agreement has been approved by the IRS (see below), the application generates filing instructions indicating that the electronic funds withdrawal amount equal to the
Amount of tax payment (Force)
field on the ELF screen and a check payment of the difference between that amount and
Amount paid with the tax return or notice
on the 9465 screen.
Enter the amount of each payment to be made monthly. Monthly payments cannot be less than $25 if filing the return electronically. This field is mandatory.
Enter the day of the month that the payment will be made. Do not enter a day later than the 28th. This field is not required if payments will be made by payroll deduction.

to have the monthly payments automatically made by electronic funds withdrawal from a bank account. In addition, use the Bank screen to designate which account to use by completing the bank account information in the Direct Deposit/Electronic Funds Withdrawal Information section.

in this field to fill out Form 9465 line 14 and print Form 2159, Payroll Deduction Agreement, with Form 9465. Form 2159 prints in triplicate and must be completed by the employer and attached to Form 9465 before it is submitted to the IRS.
When submitting Form 9465, the application prints filing instructions and the 1040-V with the total amount due shown on the tax return. Custom paragraph

can be selected to explain the process of requesting an installment agreement using Form 9465, but the default filing instructions will show the total amount due on the return because the installment agreement may not be accepted or the taxpayer may decide to avoid fees and interest and not file Form 9465. Marking this box will print the filing instructions and appropriate forms in the following formats:
  • For paper filed returns, the Form 1040-V (if not suppressed) will print with the
    Amount paid with the tax return or notice
    field on the 9465 screens, and the filing instructions will indicate to mail a check for the same amount.
  • For electronically filed returns with no electronic funds withdrawal, the Form 1040-V (if not suppressed) will print with the
    Amount paid with the tax return or notice
    field on the 9465 screen, and the filing instructions will indicate to mail a check for the same amount.
  • For electronically filed returns with an electronic funds withdrawal for the complete amount on Form 9465 line 8, no Form 1040-V will print and the filing instructions will indicate the electronic funds withdrawal amount with no check required.
  • For electronically filed returns with an electronic funds withdrawal less than the amount on Form 9465 line 8, Form 1040-V (if not suppressed) will print with the difference between the electronic funds withdrawal amount entered in the ELF screen in the
    Amount of tax payment (Force)
    field and the total amount paid with the return entered in the
    Amount paid with the tax return or notice
    field on the 9465 screen. The filing instructions will indicate the amount of the electronic funds withdrawal, and to mail a check for the amount on Form 1040-V.
    The Form 9465 custom paragraph can be used in conjunction with this option. This field will automatically be marked if the
    Approved through Online Payment Agreement Application
    field is marked.
If the taxpayer has been approved for an installment agreement through the IRS Online Payment Agreement, Form 9465 is not required. This box can be checked to print the filing instructions to reflect the agreement, without printing Form 9465. The filing instructions will print as described in the
Print remittance information...
field to reflect installment agreement payment with return. Appropriate data entry on the 9465 screen must be completed to indicate the amount paid with the return, the amount to be paid monthly, the day of month the payment is to be made, and if monthly payments are to be made by electronic funds withdrawal. Bank information is entered on the Bank screen for electronic funds withdrawal. An electronic funds withdrawal can be set up in the ELF screen in the
Amount of tax payment (Force)
field for the amount paid with the return. The Form 9465 custom paragraph will print with the text, "Your installment request has been approved by the IRS."

Form 9465 Part II - Additional Information

Use this section to enter information to complete Form 9465 Part II. This part is required only when the taxpayer has defaulted on an installment agreement within the past 12 months, the amount owed is greater than $25,000 but not more than $50,000, and the amount on Form 9465 line 10 is greater than or equal to the amount on line 11a or 11b. If the amount owed is greater than $50,000 or the amount on line 10 is less than the amount on line 11a or 11b, Form 433-F, Collection Information Statement, is required.
Enter the amount paid for health insurance premiums per month only if the premiums are not paid by payroll deduction.
Enter the amount paid for court-ordered payments per month only if the payments are not made by payroll deduction.
