Form 56 General Information & Authority

If the taxpayer is filing as the fiduciary, enter

. If someone other than the taxpayer or spouse is filing as the fiduciary, enter the fiduciary's name in

Enter the identification number of the person for whom you are acting.
If acting on behalf of a decedent, enter the decedent's Social Security number as shown on their Form 1040.
Enter the address of the person for whom you are acting. Include the suite, room, or other unit number after the street address.
The application updates this field with the taxpayer's or spouse's name based on the entry in
T, S
on the
56
screen. If someone other than the taxpayer or spouse is acting as the fiduciary, enter that person's name.
Enter the address of the fiduciary filing Form 56 if different from the address entered in the
1040
screen.
Enter the telephone number if different from the home/evening telephone number entered in the
1040
screen.
Select the code in this field from the list of available options to indicate the authority for the fiduciary relationship.
