Screen 8886-2 - Reportable Disclosure Statements, Page 2 (1040)

Overview

Use this screen to enter information to complete Form 8886, Reportable Transaction Disclosure Statement, Page 2, line 8.

Entity Involved in the Transaction

Use this section to enter information to complete the line 8 identity information and descriptions for the entities and individuals involved in the reportable transaction.
Select the corresponding reportable transaction unit from the drop-down list. If more than one entity is involved in the corresponding reportable transaction, add additional units of the 8886-2 screen and associate the additional screens with the corresponding reportable transaction.
Select the type of individual or entity involved in the transaction from the drop-down list. The corresponding box is marked on Form 8886, Page 2, line 8.
Enter the identifying number, name, and address of the entity or individual involved in the transaction. If the entity or individual has a foreign address, enter a period ("
.
") in the
State
 field and leave the
Zip code
 field blank.
If the entity or individual has a foreign address, select a foreign country code from the drop-down list. Based on your selection, the foreign country name displays automatically. Enter the foreign province or country and foreign postal code.
Enter a brief description of the entity's or individual's involvement in the reportable transaction. For each foreign entity, identify its country of incorporation or existence. For each related entity, explain how it is related.

 if you do not want this particular individual or entity involved in the reportable transaction to proforma to the following year.
