Form 4136 - Computation of Credit for Federal Tax on Fuels
Registration number
Enter the registration number for registered ultimate vendors of certain types of fuel. The registration number reports on the appropriate line of Form 4136.
Explanation of evidence of dyes
In the statement attached to this field, enter the required explanation for the evidence of dyes. When this statement contains data, the application marks the appropriate box on Form 4136 and generates a printed statement to include with the return.
Gallons / Type of Use
Enter the number of gallons of fuel in each rate category field. Enter only whole gallons because the application does not handle fractional amounts. A list is available for other nontaxable use fields. When you choose a type of use from the list, the number reports on the appropriate line of Form 4136.
Statement instead of previously filed certificate (Electronic filing only)
In the statement attached to this field, enter information to indicate that a certificate has been previously filed with the IRS. This information is included with the electronic file and you do not need to file the certificate again with the return.
