Screen IDTheft - Identity Theft Affidavit (1040)

Overview

Use this screen to complete the information necessary to file Form 14039, Identity Theft Affidavit, if the taxpayer is an actual or potential victim of identity theft and would like the IRS to mark their account to identify any questionable activity.

Form 14039 - General Information

The application lets you enter information for up to two Forms 14039. Use this field to indicate for whom the Form 14039 is being completed. If this field is left blank, the application defaults to the taxpayer.
Use this field to select the reason for filing Form 14039. Form 14039 won't print if this field is blank.
If you selected

field, enter the IRS Notice or Letter number(s).
Enter the year for which a tax return was last filed by the taxpayer. If the taxpayer wasn't required to file, leave this field blank and select
Taxpayer is not required to file
.
Mark this box if the taxpayer isn't required to file a tax return. When this box is marked, "NRF" prints in
Last tax return filed...
on Form 14039.

Representative Information

When the representative is the preparer, you can enter representative information by accessing the
Representative
window for each preparer under
Setup
,
Office Configuration
, then
Preparers
.
The information in this window will transfer automatically to the
IDTheft
screen when the preparer's name is selected. Enter the representative's name directly or press
F4
to select from the list of available representatives, and complete the
SSN
,
Telephone number
,
Address
, and
CAF number
fields.
