Form W-7 - ITIN Application - Submission information

Use this section to indicate the reason for submitting Form W-7 and why an individual taxpayer identification number (ITIN) is needed. Use a separate W7 screen for each person for whom an ITIN is needed. A return can't be electronically filed using an ITIN in the calendar year the ITIN is issued.
To print Form W-7, you'll need to select an application type in this field.
If the reason type is

Other, enter a description in the statement associated with the
Other reason
field.
If the reason type is
d, Dependent of US citizen/resident alien
, then select the relationship. This field is not available if the reason type is anything other than reason
d
.
If an exception applies, a U.S. tax return is not filed and the Form W-7 is filed by itself. The checkbox is marked on Form W-7, line h, Other, and the exception number prints on the corresponding explanation line.
Enter the treaty country and article number if an exception to filing a U.S. tax return, and/or the reason for submitting Form W-7, is to claim the benefits of a U.S. income tax treaty with a foreign country. A custom dropdown is attached to the

field.
By default, the taxpayer information from the 1040 screen prints on Form W-7 (as the U.S. citizen/resident alien) to the right of reason

field. If that's not desired, enter information in these fields.

Name and address

Use this section to enter name and address information for the applicant.
Enter the legal name of the applicant as it appears on the documents. This should be the same name as it would appear on a U.S. tax return. The individual taxpayer identification number (ITIN) will be established using this name.
Enter the name of the applicant as it appears on the birth certificate, if it is different from the applicant's legal name.
The address in the 1040 screen transfers to Form W-7. Enter a mailing address here if the address in the 1040 screen is not the applicant's mailing address in the country where they normally reside. The IRS uses this address to send correspondence to the applicant.
If the applicant no longer has a permanent residence, enter the foreign country where they last resided. Note that nonresident aliens (filing for reason
b
) need to provide a complete foreign address.

Use this section to enter birth information.

Other information

Use this section to enter other applicant and delegate information.
Enter the country or countries of citizenship and any tax identification number given by that country, if applicable. A custom dropdown is attached to the
Country(ies) of citizenship
field.

The applicant needs to send an original valid passport (with a date of entry into the United States) or 2 other current forms of documentation to the IRS to verify identity.
If the identification code is “other,” enter the description of the document in the
Description
field.
Leave the
Entry date in US
column blank if the individual hasn’t entered the United States.
If the applicant previously received an Individual Taxpayer Identification Number (ITIN) or Internal Revenue Service Number (IRSN), use the statement attached to this field to enter the number and the name under which it was issued.
Enter information for the college/company to which the applicant is affiliated (and the length of stay in the U.S.) if code

is selected in the
Reason for submitting Form W-7
field, and the applicant is filing a U.S. tax return, is not eligible for an SSN, or is temporarily in the U.S. for business purposes. (The ZIP Code is not required.)
If the applicant is a dependent, a delegate can sign for the applicant. Enter the name of the delegate and select the code that represents the delegate's relationship to the applicant. For a court-appointed guardian, court-appointment papers need to be attached showing the legal guardianship.
