You need to submit a signed copy of the Form 5347 when submitting Form 500 electronically. Print a copy of the 5347 and have the client sign it. Scan the signed copy, then attach to the e-file using the title
Form 5347
. You may also need to attach the Letter of Testamentary and the proof of death as a separate item. See the form instructions for details on when to attach each document.
