The federal designation code transferred from federal POA Screen can differ from the state designation code. Select the applicable state designation code from the list provided as needed.
To use the enrollment card number instead of the jurisdiction on Form M-2848, delete the data in the
Jurisdiction
column of the Appointee information statement.
