Use the
Extension for shareholder consent
window to enter information for shareholders whose consent was not included on the originally filed Form 4876-A.The consents must be attached to a timely filed Form 4876-A. However, if you've reasonable cause for not filing a shareholder consent on time, you may file the consent within an extended period granted by the IRS.
Enter information here if you've received an extension for filing the shareholder consent. UltraTax CS prints the separate consents for the shareholders designated to print on extension, but does not print the Form 4876-A. You'll need to attach the separate extension consents to a copy of the originally filed Form 4876-A.
Go to the
Shareholder Information
window, select the 4876-A
tab, then Extension for Shareholder Consent

Shareholder
Select the shareholder for whom you want to print the separate shareholder consent on extension.
Shareholder consenting on extension
Mark this checkbox to print the shareholder consent on a separate statement. You'll need to mark this checkbox when the consents were not filed on a timely basis and an extension was granted by the IRS.
Person with joint ownership of stock
If the stock of the Interest Charge DISC is jointly owned by a husband and wife, both need to sign the consent. If the stock is owned by tenants in common, joint tenants, or tenants by the entirety, each person needs to sign the consent.
Enter the name of the person with whom the stock is jointly owned. This information prints in the separate shareholder consent.
Person consenting on behalf of shareholder
If the shareholder is a corporation or partnership, an officer or partner who is authorized to sign the other returns may sign the consent. The trustee should sign for a trust shareholder, and the executor or administrator should sign for an estate shareholder.
Enter the name and title of the person consenting on behalf of the shareholder. This information prints in the separate shareholder consent.
