Screen 5500 - General and Employer Information

General Information

Type of return to calculate
This field defaults to a small plan if you leave it blank.
Original return acknowledgment ID number
If you are amending Form 5500 or 5500-SF, enter the original return’s acknowledgment ID number.

Sponsor / Employer Information

Prior sponsor information, if changed
If there was a change in the sponsor’s name and/or employer identification number (EIN) since you filed the last return / report for this plan, enter the name and/or EIN of the sponsor as the information appeared on the last return / report filed.
Plan name and number
If there was a change in the plan name and/or employer identification number (EIN) since you filed the last return for this plan, enter the plan name and plan number as they appeared on the last return.
Individual signing for plan sponsor
This designates the name of the person whose electronic signature information (User ID and PIN) you enter on the ELF screen as plan sponsor. The name prints on the form, not in the actual signature field. The plan sponsor still needs to sign the return on the line above the
Signature of employer/plan sponsor
 text.
