If the business uses a foreign address, enter the city and the foreign zip code. Leave the state box blank and enter a foreign country code and foreign province or state in the relevant boxes.
Telephone number
Enter the telephone number (including area code) that public and government regulators would use to obtain information about the organization’s finances and activities. The number entered here is also used for the officer’s telephone number in the electronic file.
Year of formation
Enter the year in which the organization was legally created under state or foreign law. If the entity is a corporation, enter the year of incorporation.

If the organization’s gross receipts are normally $50,000 or less, select the code to indicate if Form 990-N should be prepared instead of Form 990.
