Enter the email address for the organization if different than reported on the federal return. All organizations must provide an email address.
Second officer signature block: Name / Title
Enter the name and title of the second officer signing either the Initial Charitable Organization Registration Application or the Renewal Charitable Organization Registration Application. The information you enter in the federal 990 and 990PF screens completes the name and title of the first officer signing the return.
Amended return explanation
When you select an option in this field, the words "AMENDED RETURN" print at the top of the main forms, as applicable. If you leave this field blank and you marked the federal return as amended in the federal 990 or 990PF screens, it is assumed that state return is amended also.
