Select the code that represents the appropriate purpose of the organization.
Organization full address
If the organization’s mailing address is a P.O. box, enter the full street address in these fields. This information displays on Form PC, Page 3.
Contact person: Name
If the contact information for Massachusetts differs from the federal information, enter the state information in these fields. This information displays on Form PC, Page 3.
Officer: Name
Enter the name and title of the officer providing the second required signature. This information displays on Form PC, Page 12.
Amended return / Explanation of changes
When you select an option in this field, the words "AMENDED RETURN"print at the top of the main forms, as applicable. If you leave this field blank and you marked the federal return as amended in the federal 990 or 990PF screens, it is assumed that state return is also amended.
