Use these fields to complete the heading sections of Forms CHAR500 and CT-13.
Designee’s: Name
The designee’s information you enter prints on Form CT-13 in the Third-party designee section. If this field is blank and you selected Yes in the
Authorize discussion with taxing authority
 field, the name of the preparer prints on the form.
Authorized person’s: E-mail address
The New York State Department of Tax and Finance mandates a limit of 30 characters for the organization mailing address for electronic filing purposes. However, federal data entry allows up to 35 characters. If the address you enter for federal purposes exceeds 30 characters, a diagnostic displays with the required data entry in these fields.
Years audited by the Internal Revenue Service
Mark the boxes to indicate that the organization has been audited by the Internal Revenue Service. This information proformas automatically from year-to-year.
Final return
If you did not mark the
Terminated
field in the 990 screen or the
Final return
 field in the 990PF screen, enter
X
 for a final year the organization has filing requirements with the State of New York. An entry here also marks the Final Filing box on Form CHAR500 and the ceasing operation of an unrelated business box on Form CT-13.
Amended return

 if you did not mark the

 field in the federal 990 or 990PF screen.
