Enter the name of the business.
Enter the mailing address if different from the federal address.
Enter the taxpayer’s name for the signature line, if different than the federal officer (1120), tax matters partner (1065), taxpayer (1040), or fiduciary (1041) in the
TYGen
 screen in Texas’s General folder.
Enter the telephone number, if different from the federal return.
Enter the fax number, if different from the federal return.
Enter the e-mail address, if different from the federal return.
If the information in the most recent rendition statement filed in a prior tax year is accurate with respect to the current tax year, enter
X
. UltraTax CS marks the checkbox in the signature area and does not show the calculations on the return.
Enter any supplemental information that should be submitted with the return.
