Enter information in the Paid Preparer section of form 94x
If you prepare Form 940, 941, 943, 944, or 945 and you're self-employed, follow these steps to mark the
Check if you are self-employed
checkbox and to populate the preparer information fields in the
Paid Preparer
section of the form.
Mark the Check if you are self-employed checkbox
The application automatically marks the checkbox only when you enter a Social Security Number (SSN) in the
Main
tab of the
Firm
screen.
Select
Setup
,
Firm Information
, then
Firm
.
Select the
Main
tab.
Enter the Firm’s SSN in the
EIN/SSN
field.
note
The application will clear the checkbox if you enter an EIN in this field.
Select
Enter
to save the firm record.
Select the preparer
Select
Setup
, then
Clients
.
Go to the
Payroll Taxes
tab, and select
Edit
.
In the
Federal 94x
grid in the
Forms
section, select
Additional Information
.
In the
94X Additional Information
window, select a staff member from the
Preparer
dropdown. The application uses the name, phone number, and other relevant information for the selected staff member to complete the forms.
note
Only the staff members you mark the
Payroll form preparer
checkbox for in the Main tab of the
Staff
screen show in the
Preparer
dropdown.
To omit the preparer signature date from the form, mark the
Suppress date
checkbox.
To include only the firm information in the Preparer section of the forms, select
Firm
from the
Preparer
dropdown.
Select
OK
to save the preparer information.
Select
Enter
to save the client record.

To update the paid preparer for multiple (or all) payroll clients, you can use the Edit multiple Clients wizard.
Verify preparer information
The
Check if you are self-employed
checkbox is marked and the
Paid Preparer Use Only
section is populated with preparer information when you print the form. Before you process and print these forms, we recommend that you make sure the form includes the correct preparer information.
