E-file error rejection codes for form NYS-45

The following error messages may happen for Form NYS-45:

State General Information/Return Signer/Title: may not be blank

State General Information/Return Signer/Phone: may not be blank

State General Information/Return Signer/Signature: X is not valid

Go to Setup
, Clients
screen, then select the Payroll Tax Information
tab.

Select the Additional Information
button for New York.

Select the preparer from the dropdown list in the New York Additional Information window.

Go to the Clients
screen, then select the Main
tab. Complete the missing information for the individual in the Additional Contacts window.

For reporting agents:

Go to Setup
, Firm Information
, then Firm
screen.

Complete the missing information in the Additional Contacts window.
