Supplemental Investment Schedules

This template can be used to determine whether supplemental investment schedules are required to accompany the financial statements and be filed with the Form 5500 for the plan under audit. This template can also be used to draft the required investment supplemental schedules.
This template is based on the Form 5500 and Instructions for Form 5500 for 2009 plan years.  Users preparing financial statements for years prior to or subsequent to 2009 should ensure that the supplemental schedules meet the requirements of the Form 5500 for those particular years. Entering Data

To determine the status of line 4i of Schedule H – Financial Information and related supplemental schedules for the plan's 2011 Form 5500, answer "Yes" or "No" to the three questions on the Requirements worksheet for the plan under audit. The Assets at End of Year worksheet appears only if you indicate in question two that the plan has assets held for investment purposes at the end of the year. The Assets Acquired & Disposed worksheet appears only if you indicate in question three that the plan acquired and disposed of investment assets during the year.
