- The SOAP workflow enables attorneys and their staff to create detailed summary reports from medical records. By applying the SOAP method, this workflow ensures that patient interactions are documented in a structured and clear manner. This enhances the ability to analyze medical information, supporting case preparation and legal decision-making with precision and clarity.
Practice tips
- In this workflow, CoCounsel conducts a 4-part structured review and analysis of uploaded medical records from a healthcare provider, including:
- Subjective analysis (summary of the patient's condition based on what the patient reports, for example, the patient's description of pain or discomfort, duration of symptoms).
- Objective analysis (summary of measurable and observable data gathered during the examination, for example, vital signs, physical examination findings, results from diagnostic tests).
- Assessment (the healthcare provider's diagnoses and/or identification of potential issues).
- Plan (the proposed treatment or management strategy, for example, prescribed medications, recommended therapies, follow-up appointments, any lifestyle changes advised for the patient).
- CoCounsel then summarizes the 4 sections into a consolidated SOAP report that includes a High-Level Summary and Visit Table. You can download the SOAP report, as well as the individual analyses.
- Upload your medical files. ChooseFiles from your device, select your files, thenSubmit.
- Tell CoCounsel to perform SOAP analysis. You can tell CoCounsel:
- “I need SOAP analysis of these records” or
- “Conduct a SOAP analysis of these records” or
- “SOAP the medical records I uploaded” or
- Any other query telling CoCounsel you want a SOAP analysis of the records.
- SelectStart Workflow.
- Wait for CoCounsel to complete SOAP analysis.
- Review SOAP analysis.
Limitations to keep in mind
- The SOAP workflow is designed to process and summarize the entirety of uploaded medical records using the SOAP method. It can't accept additional instructions to limit or focus its output on specific sections or details. Once the workflow is run, it comprehensively reviews all the provided information, ensuring a complete and thorough summary without the option for customization or narrowing the scope.
- The SOAP workflow may encounter difficulties when processing medical records that are unclear scans or contain extremely messy handwriting. These issues can hinder its ability to interpret and summarize the information with accuracy, potentially affecting the quality and completeness of the generated report. Clear and legible records are essential for optimal performance.
