Add States/Cities

Select Extensions
, then the State Options and Payments
tab.

Select option 3
or 5
for Alabama
.

Go to

Organizer
States
Alabama

.

Select E-file
, then Additional Information
. In the E-file Opt Out
section, mark the Print opt out form
checkbox.

Do a full recompute, then print.
