To print Form CF-1040.

Go to the Organizer
, and select Cities, Counties, Intangibles, Tangibles
, then select Michigan Cities
.

Select Michigan Cities
, then select Name of city
.

Select General Information
, then select the City in which income was earned (Mandatory)
.

Go to the Organizer
, select Payments and Extensions
, then select Extensions
.

Select the State Options and Payments
tab.

Don’t select option 1
or 2
to print the extension only.

Do a full recompute, then print.
