Form 5122 – CITY-V print requirements
To print Form 5122 – CITY-V, City of Detroit Income Tax Payment Voucher:
- Go to theOrganizer, and selectCities, Counties, Intangibles, Tangible,Michigan Cities, then selectAdd new City.
- Enter a city name, then go toGeneral Information.
- Select an option from theResidency Statusdropdown.
- SelectDetroitfrom theCity in which income was earneddropdown in theGeneral Informationsection.
- Go toTax Forms,Cities, Counties, Intangibles, Tangible,Michigan Cities,Detroit, then selectForm 5122 – E‑File Payment Voucher.
- Enter theWrite Payment Amount Here.
- Do a full recompute, then print.
If the form isn’t printing, make sure it is not suppressed:
- Go to theOrganizer, and selectCities, Counties, Intangibles, Tangible,Michigan Cities,Detroit, then selectExtension Information.
- Select theState Options and Paymentstab and select something other than1or2under theExtension Optionsection.
- Do a full recompute, then print.
