Select 40-V-Payment Voucher
.

An amount greater than 0 must be present on the amount to pay line.

To suppress the form:

Go to Organizer

, and select Oregon
.

Select Return Options
, and go to the section Print Options
.

Select Suppress print for Form 40-V
