Change form print requirements
To calculate and print Change Form, Philadelphia School Tax Account Change Form:
- Go to .
- SelectPhiladelphia Schooland select ownership.
- Select theGeneral Informationfolder. Go to theResidency Informationsection and make selections forOwnership (Mandatory)andResidency (Mandatory).
- Go to theChange Formfolder.
- In theGenerate Change Form Forsection, selectSchool Income Tax.
- InSection 1 – Contact Info and select the reason for submitting this change form, mark the correct checkboxes and enter contact information.
- Do a full recompute, then print.
