Form 355-7004-MISC print requirements
To print Form 355-7004-MISC, Massachusetts Financial Institution, Insurance or Miscellaneous Excise Extension Payment Worksheet and Voucher:
- Go toOrganizer,States, then theCommon Statefolder.
- Go toEstimates and Extensions, then theEstimates and Extensionsfolder.
- On theExtensiontab, findMassachusettsand mark the checkbox in the columnB On Extension.
- Go toTax Forms,States,Massachusetts, then theSelected Optionsfolder.
- In theMassachusetts Return Type, make sure theFinancial Institutioncheckbox is marked.
- Do a full recompute, then print.
If the form still isn't printing, make sure it's not suppressed:
- Go toOrganizer,States, then theCommon Statefolder.
- Go toPrint Options, then thePrint Suppressionfolder.
- On theSuppression MD-RItab, make sure theExtcheckbox is cleared forMassachusetts.
- Do a full recompute, then print.
