Form CF-7004EFT Application for Extension print requirements

To print the Michigan Cities form CF-7004-EFT, Application for Automatic Extension of Time to File Certain Returns:
  1. Go to
    Organizer
    ,
    States
    ,
    Common State
    ,
    Print Options
    , then
    Print Suppression
    .
  2. On the
    Suppression SC-Cities
     tab, make sure the
    State Forms
     and
    Ext
     checkboxes are cleared for the
    Michigan Cities
     row.
  3. Go to
    Organizer
    ,
    Cities
    ,
    Michigan Common Form Cities, MI
    ,
    [city name]
    , then
    Return Options
    .
  4. In the
    Print Suppression
     section, make sure the
    City Forms
     checkbox is cleared.
  5. Go to
    Extension Information
     and make sure the
    Extension
     checkbox is marked.
  6. Go to
    Bank Information
    . In the
    Payment Voucher
     section, mark the
    Filing by EFT
     checkbox.
    note
    If the
    Informational only
     field contains
    does not allow payments by electronic funds withdrawal
     or
    does not accept CF-7004-EFT payments
    , then the form will not print.
  7. Go to
    Tax Forms
    ,
    Cities
    ,
    Michigan Common Form Cities, MI
    ,
    [city name]
    , then
    CF-1120
    .
  8. On the
    CF-1120, Page 1
     tab, make sure there is an amount on
    line 15, Balance Due
    .
  9. Do a full recompute, then print.
