folder.

Expand the Credits
folder and select Form 323
.

In Part I
, ensure questions 1 and/or 2 are answered YES
.

Ensure the value is greater than 0 on Page 1, Part III, Line 9
.

If the form doesn’t print, check if it's suppressed:

, and select Return Options
.

In the Print Suppression section, make sure the checkbox for Form 323
is cleared.
