To print Form BIRT/BIRT-EZ:

Go to

Organizer
States
State and City Activation

Select State and City Activation
and go to the Cities
tab.

Mark the Philadelphia
checkbox.

Go to

Organizer
Cities
Philadelphia, PA

Select General Information
, then select BIRT
or BIRT EZ
.

To suppress Form BIRT/BIRT-EZ:

Go to Organizer
, Cities
, then select Philadelphia, PA
.

Select Return options
and go to the Print Suppression
section.

Select the City forms
checkbox.

If Philadelphia
is selected in the Force Print
section, the forms will always print.
