To print Form 6Y, Wisconsin Modification for Dividends.
  1. Go to
    Organizer
    ,
    General Information
    , and select
    Basic Return Information
    .
  2. On the
    Entity Information
     tab, in the
    Return/Entity Type
     section, the
    Return type
     needs to be
    1120 - U.S. Corporation
     and the
    Entity type
     needs to be
    Top Consolidation
    .
  3. Then go to
    Tax Forms
    ,
    States
    , and select
    Wisconsin
    .
  4. Select
    Combined Report
    , then
    6Y - Dividends
     and make sure there is an amount on line 4.
