This rejection occurs when an Illinois 1065 e-file includes direct debit bank information without an account holder name.

FormILFinancial StatePayment/AccountHolderName is required

and enter the account holder name.

Complete all columns to ensure all bank information is complete.

Do a full recompute, recreate the e-file, and resubmit the return.
