View Questionnaire Results
If the
Name
field on a Questionnaire is clicked, the system will redirect to the View Questionnaire
screen. From there; all responses, notes, and attachments for the Send To
company’s name will appear.
You can also set Questionnaires to a failed, pending, or approved rating. Selecting Failed or Approved from the
Rating
drop-down box locks the Questionnaire so partners can make no further changes.
The top section of this screen shows high level information regarding the specific Questionnaire, including a Best Practices (BP) count, and (if the Questionnaire is complete), the final vulnerability score.

View Questionnaire
also enables you to send reminders to partners as well as generate a .pdf or Print View
of a Questionnaire from the links (top of screen).
View Question Response
To see the details of a response to a specific question, click the
Name
field.

Corrective Action Management
If you click a
Yes
link in the Corrective Actions Sent
column of the View Status
screen, the View Corrective Actions
screen will open. This screen displays all Corrective Actions for the business partner regarding the specified Questionnaire as well as high level header information.The matrix below the header information provides a list of each triggered Corrective Action; including the
Question Number, Question Name, Question Category, Responsible Party, Due Date, Reminder Date, Vulnerability, Status
and Rating
.Columns can be sorted by clicking column headers so Corrective Actions can be prioritized as desired (e.g. most severe vulnerability). The arrow to the left of each Corrective Action on the grid can be clicked on to expand the grid to show the most recent communication on that Corrective Action.

To trigger a reminder.
- Choose a Corrective Action from the grid and check its corresponding box to the left.
- ClickSend Reminderin the top left corner.

To resend a Corrective Action.
- Choose a Corrective Action from the grid and check its corresponding box to the left.
- ClickResendin the top left corner.

It is possible to CC additional e-mail addresses when sending out Corrective Action (CA) reminders/resends. This ensures all relevant contacts are included in communications. The
Reset
link deletes the selected CA records.
Each triggered Corrective Action from a specific Questionnaire for a partner will appear on this screen. The
Question Name
links the user to the specific Corrective Action follow-up screen.
The
Corrective Action Followup
screen is used to review more specific details regarding the Corrective Action, the Partner/Supplier’s response(s) and closing Corrective Actions once deemed appropriate to do so (e.g. when the partner/supplier has responded with the appropriate action). Partners complete the Action Taken
and Responsible Party
fields. Once they have done so, those responses will be reflected here.The
Status
of a Corrective Action is determined by various actions initiated in the system by both the User and their Suppliers as follows:Status | Description |

Not Started | This status will be reflected when the Corrective Action is initially sent – whether that occurs via the Auto-Send feature or via theCorrective Action Report screen. |
In Progress | This status will be reflected when a Partner/Supplier takes any action on the Corrective Action and clicks the Save button (e.g., adding a response, uploading an attachment, entering a Responsible Party). |
Completed | This status can only be initiated by the user and will occur when the user sets the Corrective Action Approval status to eitherApproved orFailed . The Partner/Supplier does not have the ability to set a Corrective Action status toComplete . |
To continue dialogue on a specific Corrective Action or edit the text of the Corrective Action, click
Click here to Edit
next to the Corrective Action Required
link. This will allow the you to provide additional dialogue to the original Corrective Action that was sent.
A text box will open, allowing you to provide an updated message to the partner. Once that message is entered, click
Edit Corrective Action
.
Once a partner responds, update the status in the drop-down
Approval
box. Select Approved
or Failed
. Selecting and saving one of these options locks the Corrective Action from any further edits by the partner and changes the Corrective Action to Complete
.
You can change the vulnerability based on the response to the Corrective Action in the
Vulnerability
drop-down box. In doing so, the system considers the new level and recalculates the overall vulnerability score for the partner.
A communication grid in the
Action Taken
section can be used to capture history of ongoing dialogue during the Corrective Action process. The grid shows the response date/time, partner, user and message.To add a response.
- ClickAdd Responseopen the pop-up.
Responses are ordered by date/time, with the most recent response at the top. - Write the response in the provided text box.
- ClickSave Response.

Clicking
Save Response
in the pop-up dialogue box saves the response to the grid. There is no need to click Save
again on the Corrective Action Follow-up
screen.The grid can be adjusted for page size and to
View All
if needed.
