Form 204 print requirements

Refer to the following instructions to print Arizona Form 204.

To calculate and print Arizona form 204 Extension Application:

Select Organizer
, States
, then Add States/Cities
.

Select a residency status other than B-None selected (Default)
.

Select Organizer
, Payments and Extensions
then Extensions
.

Select the State Options and Payments
tab, and make sure you've selected an option for Arizona.

Alternatively, you can do the following:

Select Tax Forms

, then Arizona

Make sure the Resident 204
question beginning with "Federal extension will be used to file this return
" isn't clear.
