To generate and print Form OL-3, Kentucky Louisville Metro Revenue Commission Occupational License Tax Return:

  2. Select
    KY, Louisville/Jefferson
    , then
    General Information
    .
  3. In the
    General Information
     section, select
    Compute Form OL-3
    .
  4. Do a full compute and then print.
