To print form NJ-630-V, New Jersey Employee's Claim for Credit For Excess UI/WF/SWF, Disability Insurance, and/or Family Leave Insurance Contributions for Calendar Year 2024:
  1. Go to
    Organizer
    ,
    E-file
    ,
    Enable/Create
    , and select the
    Extension
     tab at the bottom of the page.
  2. Make sure you mark the checkbox in
    Row 44
    Column B
     to
    ENABLE
     the extension for
    New Jersey
    .
