To print Form TR-579.1-IT, New York State Taxpayer Authorization for Electronic Funds Withdrawal:

Turn on the e-file for the extension:

In Organizer
, select E-file
, then Enable/Create
.

Select the Extension
tab.

Select the Enable
checkbox in column B
for New York
.

Turn on the extension print:

In Organizer
, select Payments and Extensions
, then Extensions
.

Select the State Options and Payments
tab.

Select option 1
, 2
, 3
, or 4
for New York.

Turn on the ERO certification:

In Organizer
, select States
, then New York
.

Select E-file
, then Additional Information
.

In the Person(s) Electronically Filing Extension (Mandatory)
section, select ERO
.

In the ERO Certification
section, select the I have read the certification above and agree
checkbox.

In Tax Forms
, select States
, then New York
.

Select Resident
or Part-year Resident/Nonresident
.

Select TR-579.1-IT - Elec Fund Withdraw Auth
.

Make sure there is an amount or NONE
in line 1
.

Do a full recompute, then print.
