Form 511-V print requirements

To print Form 511-V, Individual Income Tax Payment Voucher.

Go to Organizer
, E-File
and select Enable/Create
.

Oklahoma E-File must be enabled.

Then go to Organizer
, States
and select Common State
.

Then select Bank Information
.

An amount greater than 0 has to be present on the amount to be debited.

To force print the form.

Go to Organizer
, States
and select Oklahoma
.

Then select Return Options
and go to the section Print Options
.

Then select Force Print Form 511-V
.
