Form OR-529 print requirements

To print Form OR-529, Oregon College Savings Plan Direct Deposit and Account Creation Election:
  1. Go to
    Organizer
    ,
    States
    , and select
    Common State
    .
  2. Go to
    Bank information
     and on the return tab, select
    Authorization to Debit funds
     in
    Column B
    .
  3. Select the
    Educational Savings Account
     tab.
  4. Fill in the information for the Account type, portfolio number, account number and the amount in
    columns Q
    ,
    R
    ,
    S
    ,
    T
     and
    U
    .
  5. Do a full recompute, and then print.
