To print Form 58-EPV Electronic Return Payment Voucher, make sure North Dakota isn't suppressed.
  1. Go to

     and select
    States
    Common States
    .
  2. Make sure the checkbox for
    North Dakota
     is cleared in the Print Suppression section, State Forms column.
  3. Go to
    Tax Forms
    States
    North Dakota
    .
  4. Select
    Form 58-EPV
     and make sure there's a value in
    Amount of payment
    .
