  1. Go to
    Tax Forms

    , and open the
    Calendar Year Return
    folder.
  2. Expand the
    Credits
     folder and select
    Form 312
    .
  3. In
    Part I
    , ensure any of
    Questions 1
     through
    3
     are answered
    YES
    .
If the form doesn't print, check if it's suppressed:
  1. Go to
    Organizer
    ,
    States
    ,
    New Jersey
    , and select
    Return Options
    .
  2. In the
    PRINT SUPPRESSION
     section, make sure the checkbox for
    Form 31
    2
     is cleared.
