To print, calculate, or suppress Form CT-33M, Insurance Corporation MTA Surcharge Return.

  2. Select

     tab, then in the
    Return/Entity Type
     section, for
    Return Type
     select
    Insurance
    .
  3. To update, select the
    Return/Entity Type Conversion
     tab, and then select the return and entity type.
  4. Select
    Organizer
    , then
    States
    , then
    New York
    .
  5. Select
    General Information
    , then the
    General Information
     section.
  6. Go to
    Do you do business, employ capital, own or lease property or maintain an office in the Metropolitan Commuter Transportation District?
     and select
    Yes
    .
  7. Do a full recompute and then print.
