Cause

Has a Form 1040NR with a Form 1042-S that has income.
Uses income codes 17, 18, 19, 20, 23, 25, 26, 27, 32, 35, 38, 40, 41, 42, 43, 44, or 54.
The income isn't reported on the 1040NR, Page 4, on the Dividend Equivalent line or on Schedule C.
