Activate a state or city to a return
Use the State Activation spreadsheet to add states and cities for a single company except for four states that have their own insurance returns: Michigan, Mississippi, New York, and Oregon. The spreadsheet indicates these states in column G, Insurance Return Form Type, with the entry of Insurance in the cells.
