Enter or select the appropriate data for each field in the groups of data below:

File creation

Select either Filing Only or Filing with Payment. The default is Filing Only. If you select Filing with Payment, you must enter Payment Information below.

Mandatory - Enter the username created on the AZTaxes website.
AZTaxes E-Signature PIN
Mandatory - Enter the E-Signature PIN authorized for the AZTaxes website.

Mandatory - Enter the first and last name of the person to contact for resolving any questions or problems with the electronic filing.

Mandatory - Enter the title of the person to contact for resolving any questions or problems with the electronic filing.

Preparer information

Preparer Name and Email Address
Enter the name and email address for the tax return preparer.
Preparer Firm Employer Identification Number
Enter the EIN of the firm preparing the tax return.
Preparer Firm Name and Address
Enter the name and address of the firm preparing the tax return.

Payment information

Select the appropriate account type.

Bank Routing Number
Enter the bank routing number.
Bank Account Number
Enter the bank account number.
Payment Request Date Automation
The application will compute the Payment Request Date automatically each reporting period by your selection of one of these options. The payment request date will compute equal to the return due date (i.e. the 20th of the month) or the day of the month you specify.

Override options

Select to override all diagnostics and generate the electronic tax return file.


We recommend that you review and clear each diagnostic rather than use this override. Using this override can result in your upload file not importing correctly.
