- Form W-2, Box 14, Codes 5 (State Disability Insurance Withholding (SDI)) and 8 (WD/UI/HC Withholding) with the following postal codes: CA, NJ, NY, RI, and WA will be included in Schedule A, line 5 per Schedule A Instructions.
- The statement won’t display or print if Schedule A isn't required to be filed. You can temporarily force Schedule A to print by marking2 = Itemizedin theStandard/itemized deduction (force)field on the1040screen in theGeneralfolder.
