Form W-2, Box 12, Code C - Cost of group term life insurance coverage over 50,000
Boxes 1, 3 and 5 of Form W-2 will include the cost of group term life insurance that is more than the cost of $50,000 of coverage, reduced by the amount the employee paid toward the insurance. This amount is also shown in Box 12 with a Code C. The amount reported in Box 12 does not carry anywhere on the return; it is informational data only.
