Form 8802 - Applicant's General Information

Use this section to enter applicant's general information for Form 8802.
T, S
Use this field to designate for whom the Application for United States Residence Certification is being completed.
note
  • Enter code T to generate the Application for United States Residency Certification for the taxpayer.
  • Enter code S to generate the Application for United States Residency Certification for the spouse.
Requesting additional Form(s) 6166
Enter X if the applicant is submitting Form 8802 to request additional Form 6166 certificates.
Foreign claim form is attached
Enter X if the applicant is including a foreign claim form.
Electronic payment confirmation number
Enter the e-payment confirmation number. Either the Agency Tracking ID or the Pay.gov ID is acceptable as the e-payment confirmation number.
If different from Screen 1040
Use these fields to override the applicant's first and last name, identification number, spouse's first and last name, and spouse's identification number if different from the 1040 screen.

  • The applicant's name and TIN must match exactly as they appear on the U.S. return filed for the tax periods for which certification will be based.
  • If a joint income tax return was filed for a tax period for which certification will be based, enter the spouse's name and TIN exactly as they appear on the return filed.

Enter 1 if the Form 8802 is a joint application.
If this field is left blank, the Form defaults to the applicant.
Name and taxpayer ID on the certification if different
Use this field to enter the applicant's name and taxpayer identification number as it should appear on the certification if different from the applicant's name and identification number.
Applicant's address for the certification year if different from Screen 1040
Use this statement to override the address for the certification year.

Mailing address for Form 6166
Use this statement to indicate the mailing address. Form 6166 and any correspondence will be mailed to this address. If the mailing address entered in the statement is for a 3rd party appointee, provide written authorization on line 3b for the IRS to release the certification to the 3rd party. Use the Form 8802 instructions for more information.

Use the statement or field and enter X to indicate the applicant type.

  • For Individual applicant, use the statement and enter
    X
    to indicate the status. There are 6 individual statuses: U.S. citizen, U.S. permanent resident, Sole proprietor, Other U.S.-resident alien, Dual-status U.S. resident, Partial-year Form 2555 filer.
  • Other U.S.-resident alien: If the applicant is an other U.S.-resident alien, use the statement to enter a type of entry visa, current nonimmigrant status, and date of change. Use the Form 8802 instructions for more information.
  • Dual-status U.S. resident: if the applicant is a dual-status U.S. resident, use these fields to enter the dates that correspond to the beginning and ending of the period the taxpayer was a resident of the United States.
  • Partial-year Form 2555 filer: If the applicant filed a Form 2555 that covered only part of a year for which certification is requested, use these fields to enter the dates that correspond to the beginning and ending of the period the taxpayer was a resident of the United States.

Reason: Use this statement to indicate the reason the applicant was not required to file a U.S. income tax return.
