Form 8822 - Change of Home Mailing Address

If you are a representative signing for the taxpayer, attach a copy of your power of attorney to Form 8822. For more information, see Form 8822 and Form 8822-B - Change of address FAQs.
Taxpayer prior name
Enter the prior name in this field if the taxpayer changed their name because of marriage, divorce, or other life events.

To prevent any processing delays be sure to notify the Social Security Administration of the new name to ensure the permanent records match the name on the tax return.
Spouse prior name
Enter the prior name in this field if the spouse changed their name because of marriage, divorce, or other life events.

Treat mailing address on the 1040 Screen as:
If you enter code 1 or code 2 in this field, the new address information entered on the 8822 screen in the New mailing address field will proforma to the 1040 screen next year.

The application uses the old mailing address entered in the 1040 screen or, if different, the address entered in the 8822 screen to determine where to file Form 8822.
